I went to do a "Goods In" for a purchase order and it pulled up a form with 6 of the items duplicated. 6 of the purchase items had 2 rows (same entry). 2 others were just single row (correct).

If you're sure that the Purchase Order is correct and that it's the 'Goods In' form that's doing it wrong then I don't know what to say. Nobody ever reported that before.
Does it happen on other purchase orders or is it a one-off occurrence?
I'd be happy to take a look at your database to see if I can figure out what's gone wrong. If you're happy for me to to do that then make a backup and send the backup file to:
